Notice on special rectification of collapse accidents such as lifting machinery scaffolds (Anwei [2013] No. 5)

In order to implement the spirit of the national teleconference on work safety, further strengthen the supervision and management of work safety in construction in the field of engineering construction, and effectively solve the prominent problem of frequent and frequent collapse accidents in construction, the safety committee of the State Council will continue to carry out in-depth special rectification in the field of engineering construction from May to December 2013 (hereinafter referred to as "special rectification"). The relevant matters are hereby notified as follows:

1、 Guiding ideology and work objectives

（1） Guiding ideology. Adhere to the policy of "safety first, prevention first and comprehensive treatment", adhere to scientific development and safety development, focus on continuing to deepen special rectification, take strengthening source control, implementing main responsibility, standardizing on-site management, strengthening scientific and technological support and tamping infrastructure construction as the main measures, practically implement all responsibilities, implement laws, regulations and standards, and enhance the guarantee capacity of construction safety production, Promote the continuous and stable improvement of the national construction safety production situation.

（2） Work objectives. Through special rectification, strengthen the organization and management of the construction site, promote the implementation of the main responsibilities of all parties involved in the project construction, further standardize the construction market order such as construction hoisting machinery and scaffold leasing, constantly improve and improve the relevant regulatory systems, effectively curb the "three violations" on the construction site, effectively prevent and resolutely curb the occurrence of major accidents, Strive to achieve the goal that the number of collapse accidents and the number of deaths in the construction industry in 2013 will continue to decline, with the number of major accidents falling by no less than 3.2% and the number of deaths in construction accidents falling by no less than 1%.

2、 Key points of special rectification

All regions and relevant departments shall, in combination with the actual conditions and respective responsibilities of their respective regions and industries, on the basis of conscientiously summarizing the special rectification work last year, focus on the key points, highlight prevention, and focus on rectifying the following potential safety hazards and illegal operations in various links such as various lifting machinery, bridge erecting machines, scaffolds, and formwork support systems in the project construction field:

（1） The system of shift leader of the enterprise and the project leader, supervision on-site, equipment "fixed personnel, machine, post and responsibility" is not perfect or not implemented in place;

（2） The hidden danger investigation and treatment work system is not perfect, the responsibility is not clear, the measures are not implemented, and the rectification is not in place;

（3） The lifting machinery fails to be inspected as required or fails to pass the inspection, and the special scheme and approval are not formulated for its installation, disassembly (jacking) operation, the operation process is not strictly implemented in accordance with the scheme and relevant technical regulations, and the daily maintenance is not carried out as required;

（4） The scaffold erection fails to formulate the construction scheme and approval as required, fails to erect and dismantle according to the construction scheme and standard procedures, fails to meet the requirements of the regulations and schemes, and the steel pipes and fasteners are not inspected or unqualified after the inspection;

（5） Incomplete operation procedures or construction schemes, lack of pertinence and operability, chaotic site management, illegal operation, illegal command and violation of labor discipline;

（6） The principal of the construction enterprise and other three categories of personnel have no safety production assessment certificate, the special operation personnel have no operation certificate, and the construction personnel have not received the three-level education and training or the education and training are in a formality;

（7） Failure to learn lessons seriously after the accident, failure to implement rectification measures, and repeated occurrence of similar accidents.

3、 Main tasks and measures

（1） We will strengthen source control and improve the market supervision mechanism. All regions and relevant departments should make overall coordination, source management and joint supervision, establish and improve the access system of leasing and installation enterprises, strengthen the qualification management of employees, and improve and strictly implement the system of "assigning personnel, machines, posts and responsibilities" in view of the problems exposed in the leasing, installation, use, removal, inspection and testing of lifting machinery in the field of engineering construction in recent years. Strengthen the control over the production and sales of construction materials, prevent the unqualified, defective, scrapped lifting machinery, serious bending deformation, corrosion and other inferior steel pipes and fasteners from entering the market and the construction site, and eliminate the potential safety hazards from the source.

（2） Strengthen safety management and supervision, and implement the main responsibility of enterprises. The construction unit shall carry out safety investment according to the provisions and strengthen the process supervision on the implementation of safety production responsibilities by all parties involved in the construction. The construction enterprise shall implement the main responsibility of safety production, establish and improve the special rectification self inspection and rectification account, and form closed-loop management. The supervision unit shall conscientiously perform the safety supervision responsibilities during the operation of lifting machinery and scaffold on the construction site. All regions and relevant departments shall conscientiously perform the safety supervision responsibilities for construction lifting machinery and scaffolding, strengthen supervision and inspection, and seriously investigate and deal with serious violations or major safety hazards, and punish them according to the specified upper limit; Those who disobey the supervision and law enforcement shall be ordered to stop work for rectification according to law.

（3） Standardize the management of construction site and strictly investigate and deal with the "three violations" of operators. The construction unit shall strengthen the management of the whole process of project implementation and refine the on-site management measures. The supervision unit shall strictly review the special construction scheme and conduct on-site supervision on the construction procedures with high risks. The construction enterprise must prepare and perfect special construction plans for construction hoisting machinery and scaffold erection, strengthen the control of hazard sources in all links of construction hoisting machinery and scaffold works, timely investigate and deal with various potential accidents, carefully implement various rules and regulations, and effectively eliminate the "three violations" on the construction site.

（4） Strengthen scientific and technological support and improve the technical level of construction safety. It is necessary to strengthen the promotion of new technologies such as formwork and scaffold erection and informatization monitoring in the construction industry. Starting from the production equipment and technology, enterprises are encouraged to actively apply advanced scaffold erection technology and stereotyped products to improve the level of construction scaffold instrumentalization and stereotyped. Intensify efforts to promote the application of the safety monitoring and management system of bridge erecting machine, general gantry crane and tower crane, monitor the operation of the above cranes in real time, focus on improving the accident prevention and early warning level during operation, and reduce accidents caused by illegal operation, overspeed and overload.

（5） We will strengthen education and training and strive to promote the standardization of work safety. It is necessary to further improve the staff safety education and training system, and focus on improving the quality and level of all staff training, skill training and key post training. Strengthen the training, assessment, supervision and management of front-line operators, especially scaffolders, lifting machinery installation and disassembly workers and other special operators, and strictly ensure that relevant special operators work with certificates. Front line employees who have not received training or failed the training assessment shall not be allowed to work. Accelerate the establishment of enterprise safety production standardization, and include the safety management of construction hoisting machinery and scaffold into the important content of the construction enterprise safety production standardization construction and assessment, so as to promote the enterprise to improve the safety conditions and improve the management level.

4、 Job requirements

（1） Strengthen leadership and implement responsibilities. All regions and relevant departments should fully understand the importance of deepening the special rectification work, carefully organize and make careful arrangements, establish a special rectification work leading group, clarify the division of responsibilities, promptly study and solve the key and difficult problems in the special action, and earnestly perform the supervision responsibilities. All provincial safety committees and their offices should strengthen guidance, coordination, supervision and inspection, strengthen coordination and linkage with relevant industry departments, timely grasp the situation, coordinate and promote the work, and do a good job in supervision and implementation. The office of the safety committee of the State Council will form an inspection team with relevant departments to supervise and inspect the implementation of special rectification in various regions, and hold on-site exchange meetings in a timely manner to promote the implementation of the work.

（2） Formulate the scheme according to the actual situation. All regions and relevant departments should formulate practical implementation plans in combination with the action of "cracking down on illegal activities and Punishing Violations", the investigation and treatment of hidden dangers, and the daily supervision and inspection, clarify the scope, focus, steps, requirements and guarantee measures of the rectification, and refine the special rectification work into different stages, and carefully organize the implementation. Please submit the special rectification plan to the office of the safety committee of the State Council before the end of May.

（3） Give prominence to key points and pay close attention to implementation. All regions and relevant departments shall timely supervise and inspect the deepening of special rectification in key areas, key enterprises and key projects, and carry out level by level inspection by means of patrol inspection, sampling inspection and mutual inspection, so as to find problems and hidden dangers in time, rectify them within a time limit, track and implement them, crack down on illegal and illegal acts, effectively correct violations and eliminate potential accidents. It is necessary to pay attention to the analysis and summary of the implementation of special rectification, grasp the positive and negative examples, strengthen warning education, and promote good experiences and practices. For the ineffective, formalistic and perfunctory special rectification work, especially those that cause accidents, it is necessary to notify and strictly investigate and deal with them according to law, and investigate the responsibilities of relevant units and personnel. The responsible units should deeply learn from the accident lessons, implement rectification measures, and ensure that all work is implemented and achieve actual results.

All provincial safety committees and relevant departments are requested to submit the summary of special rectification work to the office of the safety committee of the State Council before December 10.
